Private Teacher List addition request

Please fill out this form with the information needed so we can add you to the Private Teacher List we maintain on the PYP website as a community service.

IF you are ALREADY listed, and you want something changed, just please fill in your name and then go to the appropriate box, e.g. if you didn't have a website earlier, but you have one now and you want it linked.

Please choose a *primary* instrument, and list any secondary instruments in "Notes". Exception to this rule is if you have actual degrees in performance in more than one instrument, e.g. "multiple brass". We appreciate your help keeping this list as focused as possible!
